JAAD: First study! Risk factors for DLE to develop into sSLE
introduce
To date, no studies have analyzed risk factors
for discoid lupus erythematosus (DLE) to develop into severe systemic lupus erythematosus (sSLE) (defined as the need for hospitalization and special treatment).
A study from J Am Acad Dermatol aimed to identify risk factors for developing sSLE in patients with SLE alone or mild biological abnormalities and to form a predictive score
for this.
Screenshot of the literature
method
This cohort study was retrospective and included a total of 30 patients with DLE who developed sSLE and 134 patients
with DLE who did not develop sSLE.
Risk factors were identified from the literature, and then backward selection was used to remove some of the factors and identify the relevant variables used in the studies for multivariate analysis
.
The odds ratio
is calculated by weighting the number of variable points proportionally.
outcome
The key results are as follows:
Multivariate analysis of 12 selected variables showed that the following three variables were selected to form scores: age at diagnosis of DLE< 25 years (OR=2.
8 [95% CI, 1.
1 to 7.
0], 1 point), phototyping V.
to VI.
(OR=2.
7 [95% CI, 1.
1 to 7.
0], 1 point), and antinuclear antibody (ANA) titer ≥ 1:320 (OR=15 [95% CI, 3.
3 to 67.
3], 5 points).
<b10>
2.
A total of 54 patients with a baseline score of 0 who did not develop sSLE
.
A patient's rating of ≥6 is associated
with approximately 40% risk of development.
Conclusion outlook
The cohort of this study showed that risk factors for developing sSLE included: age at diagnosis of DLE< 25 years, phototyping V.
to VI, and ANA titer ≥ 1:320<b10>.
