Endovascular therapy (EVT) has become the standard of care for acute ischemic stroke with large vessel occlusion (LVO)
.
However, results on long-term clinical outcomes are lacking
.
The aim of this study was to assess the 5-year functional outcomes of patients with acute LVO in the anterior circulation undergoing EVT
.
The first author of this study is Professor Li Fengli from the Second Affiliated Hospital of Army Medical University (Chongqing Xinqiao Hospital)
.
The findings were presented as a poster abstract at the International Stroke Congress 2022 (ISC 2022)
.

STUDY INTRODUCTION This is a nationwide observational registry study of acute anterior circulation LVO patients aged 18 years and older receiving EVT in 27 comprehensive stroke centers in 12 provinces in China
.
The primary outcome was the proportion of patients with good function after 5 years, defined as a modified Rankin Scale score (mRS) of 2 or less
.
Secondary outcomes included the proportion of patients with good clinical outcomes (mRS 0-1), all-cause mortality, and risk of recurrent stroke after 5 years
.
MAIN STUDY RESULTS ➤ A total of 807 patients were included in the study
.
Of these, 642 patients (79.
6%) had 5-year follow-up data
.
The proportion of favorable outcomes (40.
7% vs 43.
2%, P=0.
321) and the proportions of favorable outcomes after 5 years (29.
6% vs 27.
0%, P=0.
278) were similar to those at 90 days after stroke
.
➤ The 5-year cumulative mortality rate (47.
0% vs 24.
7%, P<0.
001) and stroke recurrence rate (19.
9% vs 1.
6%, P<0.
001) were significantly higher than those at 90 days, with 66.
4% of stroke recurrence after 5 years death
.
➤ Multivariate analysis found that increasing age, residual disability at 90 days, and recurrent stroke were predictors of good functional outcomes and increased risk of mortality after 5 years
.
CONCLUSIONS: This study shows that patients with acute anterior circulation LVO who received EVT mostly had good outcomes after 5 years, but the mortality and stroke recurrence rates were also higher
.
Increasing age, 90-day residual disability, and recurrent stroke were predictors of good functional outcome and increased risk of mortality after 5 years
