Ventricular anatomy, normal variation
Transparent spacer cavity
term
image
Narrow, finger-like cerebrospinal fluid accumulation between the lateral ventricles
The size ranges from crack to a few millimeters, and occasionally > 1 cm
The fetal transparent septum is all cystic
The transparent spacer is seen in 15% to 20% of adults
The main differential diagnosis
pathology
Clinical points
(Left) crown and axial inset plots show typical representations
of transparent spacer and Verga cavity.
(Right) Axial T2WI shows that the transparent spacer is a build-up
of cerebrospinal fluid between the two lobes of the transparent septum.
(Left) Axis position T1WI shows a small transparent spacer cavity with a Verga cavity
.
(Right) Axial position T2WI shows transparent spacer variation
with the Verga cavity.
Middle sail cavity
term
image
Triangle cerebrospinal fluid cavity
The size of the CSF buildup ranges from sutured line to triangular, round/oval
Isodensity/equisignal with cerebrospinal fluid
Ultrasound shows a low echoic cyst at the midline between the hemispheres of the brain

Diagnostic outline
(Left) Sagittal bitmaps and axial bit illustrations show the middle sail cavity
.
(Right) 40-year-old woman with headache, sagittal T1WI showing cerebrospinal fluid-like dilation of the middle sail cavity (curved arrow), elevation of the vault (straight arrow), and flattened and downward movement of the veins in the brain (empty arrow).
(Left) 46-year-old woman has a headache and axial T2WI shows a typical triangular accumulation of cerebrospinal fluid (empty arrows) that open the dome to the sides (arrows
).
(Right) Sagittarial T1WI shows mid-sail cavity variation (white straight arrow), elevated dome (black arrow), flat veins in the brain (curved arrow), and extended to the tetrahydrate pool and the saddle pool (empty arrow).
