Hematoma expansion (HE) after intracerebral hemorrhage (ICH) is a therapeutic goal to improve outcomes
Hematoma expansion (HE) after intracerebral hemorrhage (ICH) is a therapeutic goal to improve outcomes
Recently, experts from the Department of Neurology at the McGovern School of Medicine at the University of Texas Health Science Center conducted a study of patients with spontaneous ICH within 4 hours of symptom onset between May 2014 and April 2020.
CT scans at baseline <1 hour, 1-2 hours, and 2-4 hours after symptom onset obtained from the prehospital mobile stroke unit were compared with CT scans repeated in the hospital after 1 hour and 24 hours later

139 patients were included
However, initial hematoma growth (volume/time to onset) was greater at earlier baseline scans (17 ml/h [9-35] vs 9 ml/h [5-23]) vs 4 over the same 3 time periods ml/h [2-7]; P<0.
Eight patients (16%) developed HE during this 1-hour interval; all of these occurred in patients with baseline imaging within 2 hours of onset (5/18 = 28% of patients with baseline imaging within 1 hour, 3/18=17% within 1-2 hours, 0/13=0% over 2 hours; P=0.

Twenty-eight percent of patients with ICH had baseline imaging within the first hour of symptom onset, and 17% had baseline imaging within 1-2 hours and HE developed within the next hour
