Recently, AJNR magazine published the following teaching case
.
Female, 29 years old, headache, blurred vision, and ataxia
.
Combined with the above figure, consider how to diagnose?
A.
Blake cyst
B.
Enlargement of the cerebellar medullary cistern
C.
Arachnoid cyst
D.
Dandy-Walker deformity
Answer: A.
Blake cyst
Blake cyst is a small arachnoid cyst malformation in the posterior cranial fossa that is thought to be a cystic expansion
of the myelin sail to the cerebellar medullary cistern due to the failure of the degeneration of the Blake pouch secondary to the median foramen of the fourth ventricle.
The classic imaging of the Blake cyst is a cyst filled with cerebrospinal fluid (white arrow in the figure) visible posterior to the lower part of the cerebellar vermise of the posterior fossa (yellow arrow in the figure), which is separated
from the cerebellar medullary cistern (red arrow in the figure).
No cerebellar vermist dysplasia is seen in this case, and Dandy-Walker syndrome is often associated with cerebellar vermise
