Omeprazole is currently one of the most commonly used proton pump inhibitors (PPIs) for the treatment of
    acid-related diseases.

    Figure 1: The chemical structure
of omeprazole can be decomposed in acidic solution due to polymerization and discoloration of sulfinyl groups (see Figure 1) in acidic solution, and the stability may be affected by various factors such as pH value, light, temperature and other factors of the solution, resulting in contraindications
to omeprazole and a variety of drugs.

This article mainly discusses drugs that have contraindications to omeprazole for injection to guide clinical rational drug use and reduce the occurrence of
    adverse drug reactions.

Omeprazole for injection + commonly used hemostatic drugs, discolored?
Clinical prescription: 5% glucose injection 100ml plus omeprazole for injection 40mg intravenous drip; 5% glucose injection 500ml plus diacetaminoacetate ethylenediamine injection 400mg, sequential intravenous infusion found brown turbid precipitation in the infusion set Moffey's dropper
    .
In order to prevent problems before they occur, we need to understand what are the contraindications to the compatibility of omeprazole for injection
    .
How to avoid contraindications? Today, let's find out
    together.

    Omeprazole for injection is contraindicated
with 75 drugs The instructions for omeprazole for injection have a very limited introduction to whether omeprazole for injection has contraindications, and many matching results are not recorded in the authoritative injection compatibility guide "432 IV Combination Guide" in China, which brings difficulties
to clinical medication consultation and daily nursing work.

The editor collected the articles published in China on the contraindications of omeprazole for injection, and summarized and sorted out the contraindications
    of omeprazole for injection.

Table 1: Eight precautions for the contraindications

    of omeprazole for injection
In order to avoid contraindications to compatibility and avoid unnecessary medical disputes, the following points should be paid attention to when clinically configuring and using omeprazole for injection: 1.
Strictly follow the instructions: before using the drug, medical staff should read the drug instructions in detail.

Pay attention to whether there are any contraindications
    to matching.
In clinical application, the same kind of omeprazole for injection of different manufacturers has different provisions in the drug label, and the drug label should be strictly implemented to ensure drug safety
    .

    2.
The two formulations are not mixed: omeprazole lyophilized preparations are divided into two formulations for intravenous drip and intravenous bolus, the former is usually added with the stabilizer disodium
edetate due to the long administration time.
Disodium edetate can form chelates with trace metal ions in solution to avoid metal ions accelerating the auto-oxidation of omeprazole; The latter has a short bolus injection time and generally does not add stabilizers
    .
When using a preparation for drip injection for bolus, due to the reduction of the amount of diluent, the pH value after preparation is too high, which is easy to cause local irritation; When the preparation for bolus injection is diluted and used for instillation, due to the low pH value after preparation and the fact that the preparation does not contain stabilizers, it is easy to cause discoloration and precipitation during preparation and use
    .
Therefore, mixing the two formulations should be avoided
    .

    3.
    Use alone without adding medicine:
the prepared infusion should not add other drugs
    .
Disposable syringes and infusion sets for the use of omeprazole for injection should be used alone and should not be in contact with other drug solutions
    .

    4.
Sequential administration should be flushed: sequential administration should be flushed with 0.
9% sodium chloride injection after infusion or infusion separately
    .
The half-life of omeprazole is 0.
5~1 hour, if intravenous drip and omeprazole have contraindications at the same time, it is recommended to infuse
3 hours after intravenous infusion omeprazole.

    5.
The dose of the solution should not be large: the volume of 0.
9% sodium chloride injection should be 100ml, such as the use of 250ml or 500ml infusion, due to the decrease in the pH value of the overall solution after configuration, the solution instability is increased, and the instillation time is prolonged, and it is easier to change color
    .

    6.
    Ready-to-use light protection:
The configured infusion is best used up within 2 hours, not more than 4 hours
    at most.
Light is also one of the reasons for the slow discoloration of the drug solution, so attention should be paid to avoid light
during the entire intravenous infusion process.

    7.
Thorough deiodine should be remembered: when using iodine-containing disinfectants such as iodine volt and iodine tincture to disinfect the stopper of the vial, it is necessary to use 75% ethanol (alcohol) to completely deiodine, otherwise, a trace amount of iodine infiltrating into the liquid will immediately chemically react with omeprazole, resulting in discoloration
    of the solution.

    8.
    Discoloration is no
longer used: omeprazole infusion for injection can no longer be used
if it is discolored, turbid or precipitated.
