Monkeypox (MPX) is a viral zoonotic disease whose pathogen is classified in the genus
Orthopovirus in the pox family Acne Virus.
Patients with MPX present with dermatological manifestations in the form of a typical progressive rash, and although the prognosis is good in most patients, there may also be other sequelae, such as the neuropsychiatric effects of
MPX.
Recently, a systematic review and meta-analysis showed preliminary evidence that monkeypox is associated
with neuropsychiatric complications, including encephalitis, confusion, and seizures.

The researchers analyzed 19 eligible studies published between 2003 and 2022 involving a total of 1512 patients with
suspected monkeypox.
The number of patients confirmed by CDC standard or PCR testing was 1031, accounting for 68.

Based on a small number of studies on the topic, the systematic review and meta-analysis found that severe neurologic manifestations – epilepsy, confusion and encephalitis – – – were present in a small proportion (<3%) of infected individuals – but could not be ignored
.
The prevalence of other neuroneurological symptoms, including myalgia, fatigue, headache, anxiety, and depression, is unknown, but several studies have found that at least half of the population has these symptoms
.
However, because these symptoms are common in both viral infections and hospitalized patients, the researchers note that their occurrence is more likely to be the body's response to a "systemic disease" than nerve damage

There is some knowledge gap
regarding hypothetical factors that affect the risk of neurological and psychiatric manifestations, including overall MPX disease severity and viral branching.
In addition, due to the lack of follow-up and assessment of the severity and duration of symptoms, it is difficult to determine whether these symptoms may be highly disabling or mild and/or transient

Potential mechanisms of neuropsychiatric manifestations of MPX include direct central nervous system infection, immune-mediated responses, and psychological responses
to disease.
In summary, a small percentage of monkeypox infected people develop epilepsy, encephalitis, and insanity, and some other mild neuropsychiatric manifestations may be common, but there is currently insufficient evidence to estimate the prevalence of these
.
There is less evidence on MPX's psychiatric sequelae, and although there have been many reports of anxiety and depression, the prevalence of these symptoms is unclear
.
This may require monitoring and prospective studies during current MPX outbreaks to assess the medium- to long-term sequelae
of the virus.
Robust methods will be needed to assess the potential causal relationship between MPX and these psychiatric nerves in the future, and more evidence is needed to explain the heterogeneity of prevalence estimates
