In these years, no one can avoid "stepping on thunder", including sugar friends
    .
Sugar friends stepping on thunder, and some other "thunder" is still essentially different, because once "into the pit", the high and low blood sugar caused by it is not a joke
    .

In 2022, family doctors and endocrinologists from tertiary hospitals did a series of diabetes science popularization
    .
At the end of the year, we screened out 10 content points that sugar friends are very easy to do wrong and that sugar friends' families pay high attention to, and "demining" their "thunder" behaviors one by one, hoping that other sugar friends can avoid
    them after watching it.

    Misconceptions about diabetes

Minefield 10-1: Young people must not have diabetes

    Prediabetes, no medication is required

In the past, it was thought that diabetes was a disease of old age, but in recent years, more and more young people have been diagnosed with prediabetes or diabetes
    .

In April 2020, the British Medical Journal published a "National Epidemiological Survey Report on the Prevalence of Diabetes in Chinese Groups" [1] showing that at present, the prevalence of type 2 diabetes in people aged 18 to 29 in China has reached 2%, 6.
3% of people aged 30~39, and about 20.
2% of young people aged 18 to 29 are in prediabetes, which means that one in five young people has impaired blood sugar regulation

Studies have shown that only 13.
9% of pre-diabetic patients receive lifestyle interventions, medications, and blood glucose monitoring [2].

People in the pre-sugar stage may feel that the blood sugar level is only slightly higher, and the body does not feel abnormal, so it is extremely wrong
    to relax blood sugar management.

Patients with prediabetes should be actively engaged in lifestyle (diet + exercise) interventions, and pharmacological interventions should be initiated if glycaemic control is not within the ideal range after six months of intervention [3].

Metformin reduces the risk of diabetes in prediabetic populations and is well tolerated and effective in the long term [4,12].

    Misconceptions about lowering blood sugar

Minefield 10-2: Superstition of hypoglycemic health care products, instruments to reduce glucose

Believe in some health experts' hypoglycemic tricks and ancestral secret recipes

Many elderly people are always very superstitious about some hypoglycemic health care products, always fantasize that the use of some instruments can achieve the effect of lowering blood sugar, and some people are convinced of some "divine doctors" posing as experts on the Internet and the "ancestral secret recipes" they hold
    .

The mood of wanting to reduce blood sugar can be understood, but "health care products can not replace medicines" This is printed on many boxes, and do not expect the so-called experts and "ancestral secret recipes", once you believe, give up taking hypoglycemic drugs, will cause large fluctuations in blood sugar, long-term so will lead to various chronic complications
    .

Therefore, hypoglycemia still depends on formal methods: lifestyle intervention, long-term standardized use of hypoglycemic drugs
    .

Minefield 10-3: High GI food, sugar friends can not stain at all

    Diet control is a problem
that diabetics must face for a long time.

But when it comes to holidays, some special foods such as rice dumplings in the Dragon Boat Festival and tangyuan in the Lantern Festival, the festive atmosphere makes sugar friends can't hold back their hunger, but sweet zongzi and sweet rice balls are high GI foods with a high glycemic index [5], do sugar friends eat it or not?

For high-GI foods, sugar lovers are best not to eat
    without eating.
If you can't help it, in the case of regularly taking hypoglycemic drugs and cooperating with exercise, you can control the total calories of the diet and eat some high-GI foods to relieve your hunger, but you still have to be in moderation and can't seize the opportunity to eat
    hard.

Minefield 10-4: If the body is not uncomfortable, there is no need to take hypoglycemic drugs

"Three more and one less" (eating more, drinking more, urinating more, and losing weight) is a typical symptom of diabetes, but not everyone [6].

Some people have been diagnosed with diabetes, but they feel that their bodies are quite normal, regardless of their mouths and legs, and do not take hypoglycemic drugs
    .

The consequences of no symptoms are more terrible, because there is no discomfort, many people should not be right, should eat and drink, suddenly one day may have a big accident, and long-term high blood sugar, damage to human organs is happening
    all the time.

Therefore, lowering sugar cannot be by feeling
    .
Since the diagnosis of diabetes, sugar friends should monitor blood sugar, take drugs regularly, stabilize blood sugar, and do not equate the body with normal
    blood sugar.

Minefield 10-5: Stop the drug as soon as the blood sugar stabilizes

Stable blood sugar is the result of the combined effect of drugs, diet, and exercise, of which drugs play a large
    role.
However, many sugar friends do not cherish this hard-won "stable blood sugar", and stop the drug
as soon as they receive some effect.

In this way, the body will face the huge fluctuation of blood sugar caused by the sudden cessation of the drug, and then it will have to be controlled to a stable state, and it will have to spend a lot of effort
    again.

As a chronic disease, the vast majority of people have to take drugs for life, and "stop when you see good" is really not advisable
    .
The truth is known, but sugar friends always have their own sayings, such as the number of times you take medicine every day, too many tablets, in fact, this situation can be replaced with long-acting hypoglycemic drugs, such as metformin sustained-release tablets, one tablet a day, convenient and trouble-saving
    .

Minefield 10-6: Occasionally missed doses, short-term cessation of hypoglycemic drugs is not a big deal

Taking hypoglycemic drugs on time and in quantity is a hypoglycemic guideline
that sugar lovers should engrave in their hearts.
However, some sugar friends will always forget and miss it for one reason or another, and people with big hearts will feel that this is not a big deal
    .
If there is really something bad to say, if it happens, it is 100% bad
    .

Moreover, people have inertia, as the saying goes, "there are two more and more, not again and again", but in fact, "again and again" will follow, more missed and forgotten to take appear, blood sugar will definitely be high
    .

Don't take any chances! Special reminder to sugar friends who travel often and like to travel, you must prepare medicine when you go out, do a good job of blood sugar monitoring, and don't slack
off because you only have a few days.

About the misconception of the classic hypoglycemic drug metformin

Minefield 10-7: It is a three-point poison of medicine, and long-term use of metformin will hurt the liver and kidneys

It is a three-point poison of medicine, and the mass base of this sentence is quite large
    .
Long-term drug friends do not rule out that some people will crazy brain supplement "how toxic is long-term medicine"
    .
Take metformin, which sugar friends often take, some people are worried that long-term use will hurt the liver and kidneys, what is the truth?

Metformin is actually absorbed through the gastrointestinal tract and enters the blood circulation, during which it is not metabolized by the liver, so it is not hepatotoxic.

When discharged, it is excreted from the urine through the renal tubules with a prototype, and about 90% can be cleared in 12~24 hours, so metformin itself will not harm the kidneys [7], but diabetic patients with impaired liver function or renal insufficiency must follow the doctor's advice and use drugs rationally
    .

If you know the history of metformin, such doubts in the hearts of sugar friends will also be eliminated
    .
It has been a hundred years since the first synthesis of metformin [8], and its clinical application has been 65 years [9], if it is unsafe, it has long been eliminated, and it will not step onto the throne
    of hypoglycemic classic drugs.

Minefield 10-8: Taking metformin has gastrointestinal reactions, so this medicine is not suitable for you

Some sugar friends feel that their stomach is uncomfortable when they take metformin, and they think that this medicine is not suitable for them, and then they will discount or even resist taking medicine
    .

Indeed, some sugar friends taking metformin will have certain gastrointestinal reactions, but most of them are transient, and they are more manifested in the initial drug stage, and most patients can gradually tolerate or disappear with the extension of taking time [9].

According to the recommendations of the Expert Consensus on the Clinical Application of Metformin (2018 Edition), metformin is generally taken from a small dose (500 mg/day), and when tolerated, gradually increasing the dose to help reduce gastrointestinal adverse reactions
    .

In addition, metformin has different dosage forms, sugar friends should "check" whether their taking method is correct, because correct taking can also reduce gastrointestinal adverse reactions
    .

Regular tablets should be taken with or after meals; Extended-release tablets are released slowly in the gastrointestinal tract and can be taken before, during, or after meals; And for better drug absorption, enteric-coated tablets should be taken half an hour before meals [9].

Sugar lovers who want to reduce gastrointestinal adverse reactions can directly choose metformin original research drug sustained-release tablets, whose sustained release technology allows the active ingredients of the drug to be slowly released in the stomach, and the gastrointestinal adverse reactions are significantly reduced, and one tablet a day is not afraid of missing the dose
    .

Metformin original research drug, that is, metformin patented drug, is an original hypoglycemic drug approved for marketing after strict clinical trials, with the advantages of the preparation process and strict supervision before and after marketing, so it has a more stable absorption rate and fewer adverse reactions
    .

Minefield 10-9: Metformin can lose weight, which is a "welfare" for "fat friends" and a "disaster" for "sugar friends"

With the deepening of research, metformin has been found to have a good weight loss effect [9].

Therefore, some "fat friends" immediately take metformin as a "diet pill" to eat, but some sugar friends are worried: I, a person who eats guanidine for a long time, will I get thinner and thinner after eating, and can my body carry it?

First of all, it must be clear that for simple obese people, metformin does not have a definite weight loss effect, and blind use may bring adverse consequences
    .

Second, the "weight loss" of metformin is an additional benefit of returning to normal weight after the improvement of glucose metabolism in overweight and obese diabetic patients, which is a "windfall"
    .

Third, research data show that metformin has little effect on body weight in normal-weight patients [10].

Therefore, body weight is not a determining factor
in whether metformin can be used.
For patients who are overweight, obese, or normal-weight, major diabetes guidelines at home and abroad recommend metformin as the preferred drug for the treatment of type 2 diabetes [9].

The Chinese Guidelines for the Prevention and Treatment of Type 2 Diabetes (2020 Edition) states that metformin should remain in the drug regimen for diabetes (Class A recommendation)
    if there are no contraindications.

Minefield 10-10: After taking the hypoglycemic drug metformin, you can drink alcohol freely

Recently, the epidemic prevention measures in various places have been relaxed, and it coincides with the end of the year, there are many gatherings and entertainment, and the Spring Festival is approaching soon.

Then, many sugar friends feel that they have been eating metformin well, and they can relax
    in drinking.

This understanding is very wrong! Alcohol can stimulate insulin secretion [11], if you take metformin has the ideal blood sugar control, at this time drinking alcohol, especially a large number of fasting drinking, it is easy to induce hypoglycemia, alcoholic hypoglycemia symptoms will also be covered by drunken reactions, extremely dangerous
    .

The Guidelines for the Prevention and Treatment of Type 2 Diabetes in China (2020 Edition) clearly point out that alcohol consumption is not recommended for diabetics; Be alert to alcohol-induced hypoglycemia
    .

Even if there is metformin, sugar friends should remember "not fasting, taste and stop" when they can't get rid of it, and they should remember that "metformin is forbidden for alcoholism"
    .

Diabetes is a chronic disease, the vast majority of people need to take medicine for life, everyone on the way to reduce sugar do not lose because of the small, if because of their own misunderstanding of serious complications, it is really worth the loss
    .

If you are sick, you can only face it seriously, and don't enter the abyss because of your wrong perception, and the same is true
    for diabetes.
