    Unilateral magnetic resonance guided focused ultrasound (FUS) thalamic resection has been confirmed to have a therapeutic effect on drug-refractory essential tremor (ET).


    Magnetic resonance guided focused ultrasound (MRgFUS), also known as high-intensity focused ultrasound, accurately transmits sound energy to the mid-thalamic nucleus (considered an effective therapeutic target for tremor) to heat and ablate tissue.


    ET is usually bilateral, so unilateral treatment may not be sufficient to achieve a fully satisfactory clinical state.


    Patients who were diagnosed with drug-refractory ET and successfully received FUS unilateral thalamic resection at least 5 months ago were registered in two hospitals.

    Three main time points are defined: baseline (before the first thalamus resection), FUS1 (4 weeks) ± 14 days before the second thalamus resection) and FUS2 (6 months) ± after the second thalamus resection 30 days).

    Nine patients, age (± standard deviation) 71±6 years (61-80 years) of long-term ET patients underwent bilateral staging FUS thalamic resection.


    The adverse reactions after the second thalamus resection are mild and temporary.


    Patient's clinical outcome

    Tremors improved in all patients.


    In this open pilot study, the safety and effectiveness of bilateral MRI-guided focused ultrasound resection of the hypothalamus in the treatment of essential tremor were proved.
