The brain damage biomarkers neurofilament (NfL) and glial fibrillary acidic protein (GFAP) are specific peptides that reflect the damage of nerve axons and astrocytes in the brain.
There are few data on NfL in premature infants.
It is estimated that 15 million babies are born before 37 weeks of pregnancy each year.
Recently, studies have explored the longitudinal serum levels of NfL and GFAP in preterm infants within 15 weeks after birth, and explored the possible links between these biomarkers and neonatal morbidity and neurodevelopmental outcomes at 2 years of age.
This cohort study used data from 3 clinical studies, including 221 infants born before 32 weeks of gestational age (GA) from 1999 to 2015; the neurodevelopmental outcome of 120 infants was evaluated.

The main outcomes of the study were serum NfL and GFAP levels at 2 years of age, retinopathy of prematurity (ROP), intraventricular hemorrhage, infant development grade II and III Bayley scales, using odds ratio (OR), receiver operating characteristic curve ( ROC) analysis.

In simple terms, high NfL levels in the first week of premature infants are associated with ROP and neurodevelopmental dysplasia at the age of 2 years.
