    Background: The famous Chinese herbal formula Mufangji Decoction (MFJD) in Synopsis of the Golden Chamber has been used in China for more than 2000 years to treat cough, asthma and relieve chest pai.


    Objective: To explore the efficacy and mechanism of MFJD in the prevention of lung cancer with reference to traditional usag.


    Methods: Tumor allograft experiments and host anti-tumor experiments were used to observe the direct anti-tumor effect and indirect anti-tumor immune effec.


    The active ingredient of MFJD was obtained by UPLC-MS/M.


    Results: MFJD could significantly inhibit tumor growth in host anti-tumor experiments, but not in tumor allograft experiments, indicating that MFJD has anti-tumor immunit.


    Myeloperoxidase (MPO) was chosen as the core target because it has the highest degree value in the protein-protein interaction network and potential binding to tetrandine and dehydroxylactone in molecular docking activ.


    In vivo, MPO-expressing neutrophils are inversely associated with lung cancer progression, and MFJD promotes neutrophil-related immune surveillanc.


    Key compounds from MFJD modulate 36 lung cancer prevention targets identified by UPLC-MS/MS, network pharmacology, and transcriptomic.


    Conclusions: MFJD protects against lung cancer mainly by promoting MPO expression to maintain neutrophil immune surveillance, and its key compounds are tetrandine and dehydroxylacton.
