    Clinical question: Observational studies have demonstrated an association between gout and high blood pressure, but are they causal?

    Screenshot of the literature

    ▎Research protocol: We obtained genetic information from the Taiwan Biobank, including 88,347 participants and 686439 single nucleotide polymorphisms (SNPs).

    。 A new Mendelian randomization (MR) model was used to detect a causal relationship between gout and susceptibility to hypertension by roughening exposure after removing SNPs associated with measuring confounders, using 4 gout-associated SNPs and 10 SNPs associated with hypertension, and vice versa
    .
    Inverse variance weighting (IVW) and polygenic risk score (PRS) methods were used to estimate effect size
    .
    MR analysis of coarse exposure was performed
    with and without covariate adjustment.

    ▎Main findings:

    • Of the 88,347 participants, 3,253 (3.
      68%) had gout and 11,948 (13.
      52%) had hypertension [31.
      9% men; Average age 51.
      1 (SD, 11.
      1) years].

    • After adjusting for confounding factors measured, MR analysis of coarsened exposure found IVW [relative risk (RR) 1.
      10; 95% CI 1.
      03 to 1.
      19; P = 0.
      0077]
      and PRS method (RR 1.
      10
      ; 95% CI 1.
      02-1.
      19;
      P=0.
      0092) showed that
      gout had a significant positive causal effect
      on susceptibility to hypertension.

    • The results of causality are the same
      before and after when it comes to measuring confounders.
      However, hypertensive susceptibility has no causal relationship
      to gout.

    Conclusion and outlook:

    In this study, it was found that susceptibility to gout has a causal relationship with hypertension, but susceptibility to hypertension has no causal relationship
    with gout.
    Proper management of gout can reduce the risk of
    high blood pressure.
