China is a big country with stomach cancer, and the number of cases of stomach cancer and deaths due to stomach cancer every year is close to half of the
world's total.
Among them, Helicobacter pylori (Hp) infection is the most important controllable risk factor for gastric cancer [1].
As an individual, getting regular H pylori testing is a protection
for yourself and your family.
As a front-line doctor, mastering accurate scientific diagnosis methods and effective treatment methods is an inevitable means
to effectively control the spread of diseases.
What are the diagnostic criteria for Helicobacter pylori infection?
▎ Those who meet one of the following 3 items can be judged as Hp infection:
(1) Rapid urease test of gastric mucosal tissue, tissue section staining or bacterial culture are positive
in any of 3 items.
(2) 13C or 14C-urea breath test (UBT) positive
.
(3) Hp fecal antigen test (clinically verified monoclonal antibody method) positive
.
A positive serum Hp antibody test (a clinically validated, highly accurate reagent) indicates a previous infection, and an untreated person is considered an existing infection
.
What is the medication regimen?
Dose, usage, and evaluation of antimicrobial combination doses in the Helicobacter pylori eradication quadruple regimen:
Note: A standard dose proton pump inhibitor + standard dose bismuth (2 times/d, oral half an hour before meals) + 2 antibacterial drugs (oral after meals).
The standard-dose proton pump inhibitors were esomeprazole 20mg, rabeprazole 10mg (or 20mg), omeprazole 20mg, lansoprazole 30mg, pantoprazole 40mg, and Ilaprazole 5mg, whichever of the above; The standard dose of bismuth is bismuth potassium citrate 220 mg (standard dose of bismuth pectin to be determined); b The efficacy is graded according to Graham: grade C is 85%~89%, grade B is 90%~94%.
Except for levofloxacin-containing regimens that are not used as the initial treatment regimen, eradication therapy is not divided into first- and second-line regimens, and regimens with high efficacy should be used for primary treatment
as much as possible.
The recommended course of empiric bismuth quadruple regimen is 14 days, unless local studies demonstrate an efficacy of 10 days (eradication rate >90%)
.
After initial treatment failure, remedial treatment
can be pursued in one of the remaining options.
