In order to evaluate the accuracy of perfusion weighted imaging (PWI) in predicting the infarct volume in patients with anterior circulation and great vessel occlusion 24 hours after the onset.

    Dr.
Shashank’s team conducted a study, and the results of the study were announced at the American Academy of Neurology (AAN) annual meeting in 2021.

Research Introduction The researcher is a secondary analysis of the DEFUSE 3 study, including patients with anterior circulation occlusive stroke 6 to 16 hours after the onset.

A total of 147 patients were enrolled in this study and divided into 4 groups, of which 69 were in the untreated group, 17 were TICI 0-2a, 46 were TICI 2b, and 15 were TICI3.

The main outcome was to observe the final infarct volume (DWI-positive tissue volume) of the 24-hour MRI.

The main predictors are the baseline volume of CT/MR perfusion Tmax>6s, Tmax>10s and the low perfusion intensity ratio (HIR: Tmax10/Tmax6) at admission.

For the details of the DEFUSE 3 research, please click to view ↓↓↓6~24 hours! Because of these two studies, the time window for stroke thrombus removal is loose.

Main findings: In untreated patients, HIR (β-weight=0.
    30; p=0.
    006) and Tmax10 (β-weight=0.
    31; p=0.
    004) are indicators for predicting the volume of infarcts at 24 hours.

In patients receiving treatment, there is no consistent relationship between perfusion imaging and final infarct volume.

Research conclusions For patients with anterior circulation large vessel occlusive stroke in the late time window, HIR and Tmax10 are reliable predictors of infarct volume in untreated patients.

For patients undergoing thrombectomy treatment, further research is necessary to better understand the more complex relationship between baseline perfusion imaging and infarct volume at 24 hours and beyond.
