In order to thoroughly implement the spirit of the "Guiding Opinions of the State Council on Establishing the General Practitioner System" and "Beijing's Implementation Plan for Reforming and Improving the Training and Use of General Practitioners' Incentive Mechanism", in accordance with the "Beijing Work Plan for Improving the Quality and Scale of Family Doctors' Contracted Services" According to the requirements, our committee organized and revised the "Beijing General Practitioner Transfer Training Implementation Plan (2021 Edition)"
The "Notice of Beijing Municipal Health Commission on Printing and Distributing the Implementation Plan for Transfer Training of General Practitioners in Beijing (2019 Edition)" shall be repealed at the same time

In order to further increase the transfer training of general practitioners and meet the needs of community health work for the construction of general practitioners, in accordance with the "Guiding Opinions of the State Council on Establishing a General Practitioner System" and "Beijing's Reform and Improvement of General Practitioner Training and Use Incentives" The “Mechanism Implementation Plan” and “Beijing's Work Plan for Improving the Quality and Scale of Family Doctors’ Contracted Services” require that this plan is formulated in accordance with the actual conditions

Through systematic general practice transfer training, trainees can master the basic theories, basic knowledge and basic skills of general practice, and can use general practice clinical thinking to skillfully and standardly carry out the diagnosis and treatment of common and frequently-occurring diseases in the community , Rehabilitation and referral, management of common chronic diseases, and preventive health care work to meet the basic requirements of general practitioner positions

Clinical practitioners (assistants) who have worked in medical institutions in Beijing for at least 3 years, have obtained the qualifications of clinical (western medicine) practicing (assistant) physicians, and intend to work as general practitioners in primary medical and health institutions or general medical departments of second- and third-level general hospitals.
3.
(1) Training content
General practice transfer training consists of four parts: theoretical training, clinical rotation, series of case discussions and grassroots practice.
(2) Training time
The training time is 1 year (52 weeks in total)
(3) Training reduction and exemption
The training base can reduce or exempt the relevant training content and time according to the professional background and work experience of the trainees engaged in clinical work (the time is up to the day of training registration) according to the following circumstances

Trainees from primary medical and health institutions, who have been engaged in medical work in primary medical and health institutions for 2 years or more, and have intermediate or above professional and technical titles, are exempted from grassroots practice

Trainees from general hospitals at the second level and above who have the qualifications of attending physicians will be exempted from the clinical rotation of the second-level disciplines corresponding to their specialties (subject to the scope of registration in the physician’s practice certificate).

Trainees from general hospitals at level 2 and above, with qualifications of deputy chief physician and above, will be exempted from clinical rotation; those who have been engaged in general medical care and teaching in an independent general medical department of the hospital for 1 year will be exempted at the same time.

The registered practice scope is for non-general medicine professional training objects.
Those who have obtained the general practitioner transfer training qualification certificate can increase the general medicine professional practice scope on the basis of the original registered practice scope

Trainees from primary-level medical and health institutions with clinical work experience in general hospitals at level 2 or above can refer to 3 and 4 for the corresponding training content

Trainees who have terminated their physician practice activities for two years and need to re-register should first complete the registration, and then reduce or exempt relevant training content based on the above categories
.

The above training reduction and exemption items should be voluntarily submitted by the trainee to the education department of the unit to apply for the reduction and exemption training and details, and the unit education department coordinates the relevant department of the unit to issue certification materials and collectively report to Beijing General Medicine.
The training center will conduct an audit, and the exemption can be reduced only after passing it

Four, training methods
Adopt a modular training method, including two modules, compulsory and elective

Except that some clinical departments are rotated as elective modules, theoretical training, clinical department rotation, grassroots practice, and series of case discussions are all compulsory modules

Theoretical training segments can be centralized, remote training and other ways

Clinical rotation and grassroots practice can be carried out according to the characteristics of adult learning, combined with the previous clinical work experience of the trainees, and in accordance with the principle of "filling and filling", using a combination of clinical practice, teaching rounds, watching videos, self-study and counseling

The discussion of the series of cases was conducted by means of grouped and concentrated training to obtain credits

5.
Training base
(1) Theoretical training base

The Beijing General Practice Training Center, the School of General Practice and Continuing Education of Capital Medical University, and the institutions designated by the district health commissions and reported to the municipal health commissions to undertake relevant training work

(2) Clinical training base
.
It is a clinical training base for standardized training of resident physicians, standardized training of assistant general practitioners, and general practitioner transfer training recognized by the Municipal Health Commission

(3) Grassroots practice base
.
The basic practice base for standardized training of resident physicians, standardized training of assistant general practitioners, and general practitioner transfer training recognized by the Municipal Health Commission

Six, training and assessment
(1) Process assessment
.
The training base conducts process assessment according to training requirements
.
The focus will be on the completion of the training details of the trainees, the achievement of the credits for the series of case discussions, and the mastery of knowledge and skills
.
Assessment should include time and attendance, examination or analysis of clinical cases
.
(2) Completion assessment
.
The city organizes the completion assessment of the transfer training of general practitioners in Beijing, and the Beijing General Practice Training Center (Capital Medical University) is responsible for the specific implementation
.
Those who have completed all the learning tasks of general practitioner transfer training and passed the process assessment can participate in the completion assessment
.
The content of the completion assessment includes the comprehensive theoretical examination of general medicine and the assessment of general practitioners' clinical ability
.
Seven, safeguard measures
(1) Strengthen organization and management
.
The Municipal Health Commission is responsible for the transfer training of general practitioners in the city, organizes the formulation of training plans and training outlines, identifies training bases, and guides, monitors, and inspects and evaluates the implementation of training
.
The Beijing General Practice Training Center (Capital Medical University) is responsible for strengthening the management and supervision of daily training according to the work requirements of the Municipal Health Commission on the transfer training of general practitioners, and organizing the city's unified completion assessment, relevant information filing and training summary, etc.
Specific affairs work
.
All district health committees will organize and mobilize general practitioners’ transfer training in accordance with the construction plan of general practitioners in their jurisdictions.
In principle, the grassroots community health service centers (stations) have not yet registered (including annotations) for general medicine majors by the end of 2025.
All doctors in the scope of practice should complete general practitioner transfer training
.
The district health committee may, according to the actual situation of the district, after reporting to the municipal health committee, carry out the transfer training of general practitioners in the district according to this plan, and accept municipal monitoring and supervision and assessment
.
Each training base is responsible for implementing the training of clinical rotation modules, grassroots practice modules and case discussions, offering relevant courses and lectures to improve clinical ability, conducting daily management and process assessments for training subjects, and accepting municipal or district-level supervision and assessment
.
(2) Practice strictly in accordance with the law
.
After the trainees complete all the training content and pass the process assessment and completion assessment, the Municipal Health Commission will issue the "Beijing General Practitioner Transfer Training Qualification Certificate"
.
After obtaining the training certificate, the trainee should complete the general practice registration within 2 years
.
In accordance with the relevant national and Beijing regulations, the district health commissions will promptly change the scope of practice for those who have obtained the qualification certificate for transfer training of general practitioners, and practice in accordance with laws and regulations
.
(3) Guarantee funding input
.
General practitioner transfer training funds are diversified from the government, units and individuals
.
The Municipal Health Commission shall provide special funding guarantees for the work related to the transfer training of general practitioners undertaken at the municipal level in accordance with the annual training plan
.
The district health committee has increased its investment in improving the teaching conditions of its training bases and the construction of the teaching staff, and will guarantee the district-level special funds for the transfer training of general practitioners carried out after reporting to the municipal health committee
.
All training bases provide the trainees with necessary accommodation and corresponding living allowances during the training period, and establish a good reward and punishment mechanism to ensure the smooth implementation of daily training
.
(4) Guarantee the treatment of personnel
.
All district health committees and medical institutions should combine actual work needs, continuously improve supporting measures, improve the practice environment of general practitioners, and guide and encourage in-service clinicians of other professions to participate in general practitioner transfer training.
After completing the training and passing the assessment, they will be transferred to the full Doctor

Employers must ensure that the personnel relationship of the trainees remains unchanged during the training period, the working years are continuously calculated, and they still enjoy the wages and social security of similar personnel
