Can you tell us a little bit about pre-transplant care, during, and post-transplant care? How do I work closely with my clinician?

Chief nurse Zhu Xiaming: Patients with blood diseases often do not understand the entire transplantation process before transplantation, so transplant nurses' health education for transplant patients needs to run through the whole transplant process

Before transplantation, transplant nurses should do a good job of counseling patients, for example, explaining the concept and process of HSCT, possible adverse reactions and coping methods of patients during transplantation

In addition, transplant nurses can also lead patients to visit the transplant ward to understand the transplant environment, and can also invite patients who have received HSCT to give testimonials to improve patients' confidence
in treatment.

In transplantation, patients may experience side effects or adverse reactions, so transplant nurses have a great responsibility to provide comprehensive patient care, especially to prevent infection in patients, such as catheter care, because catheters are the "lifeline"
of transplant patients.
In addition, the patient's facial features, skin, perianal area, etc.

Once a patient has an adverse reaction or complication, the transplant nurse should promptly detect and communicate
with the doctor.

After transplantation, transplant nurses should do a good job of continuous care of patients and establish communication with patients and their families, because all the diet of transplant patients has been disinfected twice, and they need to be informed that patients should still do a good job of disinfecting the patient's diet after transplantation and discharge, otherwise intestinal infection
may be caused.

Chief nurse Zhu Xiaming: Blood system diseases, especially leukemia, mainly have four major symptoms, including anemia, bleeding, infection and tumor cell infiltration
.
Among them, the prevention and treatment of anemia, bleeding and infection has always run through the whole process of treatment, and it is important to prevent infection.

Since 1999, the Department of Hematology of our hospital has implemented the "Unaccompanied Unit" to reduce the incidence of infection and make the treatment smoother, and the infection rate of patients treated by the Hematology Department of our hospital is lower
than that of other centers.

Hematology: Hematology is an ever-evolving and constantly updated specialty, clinicians pursue better efficacy and safety, and the nursing team also strives to provide higher quality services
in nutritional assessment, patient health education, and nursing-patient communication.
Could you briefly talk about the research results of the nursing team of the Department of Hematology of the First Affiliated Hospital of Soochow University and the future research direction?

Chief nurse Zhu Xiaming: Our nursing team has won the "Civilized Gang" commendation of Jiangsu Province, and also won the "St.
Anthony-EBMT Outstanding Nursing Team" award at the 6th Sino-French Hematology Summit Forum in 2018, and also won the Jiangsu Province New Technology Introduction Award and Suzhou Science and Technology Progress Award

In terms of scientific research, our nursing team has published many SCI articles, as well as many articles in core journals, and there are still many research topics in progress

In the future, the goal of our nursing team is to do a good job in the whole process of management of the care of all blood disease patients, and we hope that this work can be incorporated into the nursing system

Yimaitong: Could you please talk about the difference in the whole process of care in the First Affiliated Hospital of Soochow University between different blood diseases such as leukemia, lymphoma and other benign blood diseases?

Chief nurse Zhu Xiaming: Leukemia, lymphoma and multiple myeloma are among the three major malignant tumors
of the blood system.
Every blood disorder is unique and not all are one-size-fits-all, for example, for patients with lymphoma, treatment requires multidisciplinary collaboration, not a care team alone
.
Patients may not be in the haematology department when they first seek medical attention, possibly because other diseases are being treated
in other departments.
For example, if a patient finds swollen lymph nodes and is diagnosed with lymphoma after pathological biopsy, the nursing team needs to work with the pathology team, including: guiding the patient to seek medical attention correctly; Do a good job of pre-health education after patients' medical treatment; Reasonably arrange the whole process of patient care, and timely notify patients to seek medical treatment; Do a good job of continuous care after discharge, and understand whether there are other problems that need to be solved by nurses after discharge; and remind patients to seek medical attention in time; Inform patients of the importance of rational drug use, can not take self-medication, should follow the doctor's advice to take medication

The same is true for the whole process of care for patients with multiple myeloma, leukemia and other blood diseases, and the nursing team should do this according to the characteristics of patients with different diseases

Yimaitong: In the past two years, CAR-T therapy has been widely used in the field of blood diseases, is there anything special about the nursing of patients receiving CAR-T cell therapy? Can you share your nursing team's nursing experience in this area?

Chief nurse Zhu Xiaming: At present, CAR-T therapy has become the main treatment method in the hematology department of our hospital, especially for patients
with refractory and recurrent blood diseases.
Some questions during CAR T cell therapy, such as when will the patient's blood cells be collected? When are the collected blood cells prepared and returned to the patient by intravenous infusion? What complications do patients have after receiving CAR-T therapy? The nursing team needs to cooperate with the doctor according to the patient's condition and clinical manifestations, and give the patient corresponding care
