    Rheumatoid arthritis (RA) is a chronic inflammatory autoimmune disease that can cause joint damage and is related to inflammation, swelling and pain
    .

    Studies have shown that the infiltration of activated immune cells (including T cells, DC cells and macrophages) into the synovium of the joints is the cause of the destruction of joints and cartilage
    .

    However, the therapeutic effects of existing drugs (such as glucocorticoids and non-steroidal anti-inflammatory drugs) are not ideal, and the efficacy depends on the development of individual diseases
    .

    In addition, strong side effects such as teratogenicity, high toxicity, and high risk of infection limit the use of high-frequency doses of these drugs
    .

     Cell delivery systems have become a promising platform for therapeutic drug delivery
    .

    Compared with traditional drug carriers, cells are an inherent part of the body and have good biocompatibility
    .

     Previously, it has been reported in the literature that platelet-derived extracellular vesicles (PEVs) can selectively target the site of inflammation and are a universal tool for diagnosis and treatment of inflammation
    .

     Based on this, in order to target the alleviation of the inflammatory response of rheumatoid arthritis, the Wang Chao research group of Soochow University, Zhou Xiaozhong of the Surgery Department of the Second Affiliated Hospital of Soochow University, and the Berlin team jointly published a report in the 11th issue of Advanced NanoBiomed Research in 2021.

    Researchers have developed a platform for platelet-derived extracellular vesicles (PEVs) and injected them into mice by intravenous injection (Figure 1a) )
    .

    Through various characterization methods, researchers found that the size of PEVs is about 140 nm (Figure 1b, c), and they have cell adhesion related proteins, such as CD41, p-selectin, etc.

    In addition, a large accumulation of PEVs in rheumatoid arthritis lesions was observed in RA mice.
    Flow cytometry data showed that PEVs selectively bind to macrophages and DC cells in the joints, further confirming the inflammation of PEVs.

    Inflammation targeting of PEVs in RA mice At the same time, the researchers loaded the small molecule of traditional Chinese medicine, berberine BBR for rheumatoid arthritis treatment.
    In the RA mouse model, compared with free drugs Intravenous injection of BBR-PEVs can significantly improve the inflammatory environment in the joint area and regulate the secretion of cytokines (Figure 4), thereby alleviating the symptoms of rheumatoid arthritis and enhancing bone density (Figure 3)
