Similar to ischemic stroke , some studies and case reports suggest that diffusion-weighted (DW) MRI may detect ischemic changes in the optic nerve in patients with ischemic optic neuropathy
Recently, a study published in the journal Radiology evaluated the impact of the time from the onset of AION to the appearance of abnormal DWI on the detection of ischemic changes in the optic nerve , providing the possibility to determine the optimal timing of diagnosis and achieve early diagnosis of the disease .
This retrospective, IRB-approved, single-center study included data from 126 AION patients and 111 optic neuritis control patients treated between January 2015 and May 2020 .
Images were analyzed individually by two radiologists who kept all data confidential .
Differences were judged by consensus consensus by a senior neuroradiologist .
The primary criterion was limited diffusion of the optic disc and/or optic nerve as subjectively assessed on the ADC map , and ADC values were measured .
Spearman's rank correlation was used to investigate the relationship between the time from the onset of visual symptoms to the completion of MRI and the diffusion limitation and ADC values .

The present study demonstrated that DWI MRI performed early after visual symptoms had good diagnostic performance for the detection of ischemic changes in the optic disc and optic nerve in patients with AION, with an optimal threshold of 5 days
