Large vessel occlusion (LVO) strokes occur in a large proportion of patients during the late window period (6-24 hours)
.
If reperfusion cannot be achieved, these patients have a poor prognosis, with an estimated mortality or serious morbidity rate of approximately 40%
Consensus on Vascular Thrombosis Since the publication of the DAWN and DEFUSE-3 trials, hospitals around the world have accumulated a large amount of data to allow the use of a variety of imaging modalities to reasonably assess clinical outcomes in patients with advanced windows selected for EVT .
We sought to perform an individual patient-level analysis of such data collected from comprehensive stroke centers around the world and to describe imaging options and outcomes in patients treated with late-present EVT .
Data from seven trials and registries were pooled for analysis
.
All patients received EVT at a late stage (>6 hours from onset to imaging)
The pooled data contained details of the following patients: patient demographics and clinical characteristics, including age, sex, and baseline National Institutes of Health Stroke Scale (NIHSS) .
Distribution of mRS at 90 days across the entire cohort and in the perfusion and collateral circulation cohorts
Of the 608 patients, 379 (62.
3%) received CTA and CT perfusion (CTP) (perfusion cohort) and 229 received CTA only (collateral cohort)
Of the 608 patients, 379 (62.
In the perfusion cohort, 77.
This proportion was higher in patients in the collateral versus perfusion group (18.
