Iditable tremor (ET) is an active neurological disorder that affects about 0.9 percent of adults and increases with age (4 percent of adults over 65).
current standard surgical treatment for idiogenic tremors (ET) is Deep Brain Stimulation (DBS).
, however, recent technological advances have led to the development of non-traumatic treatments, such as magnetic resonance-guided focused ultrasound (MRgFUS) coscchotic therapy.
method: The authors systematically reviewed the preferred reporting items based on systematic review and meta-analysis to compare the tremor severity and quality of life of one-sided MRFUS pasytherapy with one-sided and two-sided DBS therapy ET.
the PubMed, Cochrane System Evaluation Database, Cochrane Control Pilot Centre Registry and SCOPUS database.
searched these databases using the following terms: "idiomoric tremors," "idiotic tremors and abdominal intermediate nuclei," "idiotic tremors and deep brain stimuli," "idiotic tremors and high-intensity focused ultrasound."
45 eligible articles published between 1990 and 2019.
: 1202 cases were treated with DBS and 477 cases were treated with MRgFUS.
improvement in postoperative tremors was significantly better than that of MRGFUS pasythetic therapy.
subgroup analysis based on treatment bias: two-sided DBS is significantly better than MRgFUS and one-sided DBS (p-lt;0.001), but there is no significant difference between MRgFUS and one-sided DBS (p-lt;0.198).
quality of life after MRGFUS pasal therapy was significantly higher than that of DBS (p.lt;0.001).
differences in the distribution of complications between the two groups were statistically significant ( p .lt;0.001).
complications are more common in the MRGFUS group (p-0.042).
Although two-sided DBS is superior to one-sided MRgFUS masculinity in ET treatment, subgroup analysis shows that the treatment of partiality is the most important determining factor for tremor improvement, thus emphasizing the importance of future research on two-sided MRGFUS masculinity therapy.
Although the overall incidence of complications is similar for both technologies, each has a greater risk of certain adverse events, namely the electrode misplacing/problems of BS and the numbness of gait disorders and MRGFUS.
