Etanercept is a class of immune system suppressants, in addition to the use of ankylosing spondylitis (AS), what are the indications of etanercept? Let's take a look!
Rheumatoid arthritis (RA)
▍ Overview of domestic usage in adults:
Adult patients with moderately to severely active rheumatoid arthritis who do not respond to disease-modifying antirheumatic drugs (DMARDs), including methotrexate if not contraindicated, can be treated
with etanercept in combination with methotrexate.
Etanercept alone or in combination with methotrexate has been shown to reduce the progression of joint damage and improve joint function in the phase of x-ray detection [1-2].
▍Dosage:
This product needs to be used under the guidance of a specialist with experience in the diagnosis and treatment of rheumatoid arthritis and ankylosing spondylitis [1].
The recommended dose for adults (18-64 years) is 25 mg twice weekly (72-96 hours apart) or 50 mg once weekly, and a 50 mg once weekly dosing regimen has been shown to be safe and effective [1].
▍FDA dosage:
50mg subcutaneous injection, once a week [2].
Methotrexate, glucocorticoids, salicylates, nonsteroidal anti-inflammatory drugs (NSAIDs), or analgesics
may be continued during etanercept treatment.
In one study, patients with RA using etanercept twice weekly had a higher incidence of adverse effects and a similar response rate to the American College of Rheumatology (ACR), so etanercept higher than 50 mg per week is not recommended [2].
Ankylosing spondylitis (AS)
▍ Overview of domestic usage in adults:
Adult patients with severe active ankylosing spondylitis may be treated with etanercept when conventional therapy does not respond [1].
▍Dosage:
The recommended dose is 25mg twice a week (interval 72-96h) or 50mg once a week [1].
▍Review of FDA usage in adults:
etanercept is used to reduce the symptoms and signs (AS) of patients with active ankylosing spondylitis [2].
▍FDA dosage:
50mg subcutaneous injection, once
a week.
Methotrexate, glucocorticoids, salicylates, nonsteroidal anti-inflammatory drugs (NSAIDs), or analgesics
may be continued during treatment with etanercept.
In one study, patients with RA using etanercept twice weekly had a higher incidence of adverse effects and a similar response rate to the American College of Rheumatology (ACR), so etanercept higher than 50 mg per week is not recommended [2].
Psoriatic arthritis
▍ Overview of FDA usage in adults:
It is used to reduce the signs and symptoms of patients with psoriatic arthritis (PsA), inhibit the progression of structural damage in active arthritis, and improve body function
.
Enalapril can be used with or without methotrexate [2].
▍FDA usage and dosage:
the same as ankylosing spondylitis
.
Plaque psoriasis
▍ Overview of FDA usage in adults:
Etanercept is indicated for patients over 4 years of age who are ready for systemic therapy or phototherapy for chronic plaque psoriasis (moderate to severe) [2].
▍FDA dosage: Starting dose:
50mg subcutaneous injection, 2 times a week for 3 months
.
Maintenance dose: 50 mg subcutaneously 1 time per
week.
In addition to the recommended starting dose of 50 mg twice a week, a starting dose of 25 mg or 50 mg once a week is equally effective
.
The efficacy is related to the dosage [2].
