In current bladder cancer clinical practice, magnetic resonance imaging (MRI) has become an important imaging tool for locoregional staging and assessment of treatment response .
Diffusion-weighted imaging (DWI) of MRI is a functional imaging technique that shows the degree of diffusional motion of water molecules in tissues .
In bladder cancer, apparent diffusion coefficient (ADC) value, a quantitative measure of DWI signal, has been reported to correlate with Ki-67 labeling index; in addition, tumors with lower ADC values responded better to CRT .
However, there is some overlap between patients who achieve a complete response (CR) and those who do not , thus limiting its ability to predict CRT sensitivity .
Recently, texture analysis (TA) has been widely used in the qualitative assessment of various malignancies

TA can extract objective texture features (TFs) from digital medical images, whose numerical values can be used for clinical decision support, providing better diagnostic , prognostic, and predictive accuracy .
However, to our knowledge, no studies have assessed the ability of TA to predict CRT sensitivity in MIBC .

In the correlation analysis, we excluded TFs with high correlation, including the mean apparent diffusion coefficient (ADC), second quartile (Q2) ADC, gray level co-occurrence matrix (GLCM) similarity and difference, gray level Run Length Matrix (GLRLM) Short Run Emphasis (SRE) and Grayscale Zone Length Matrix (GLZLM) Zone Percentage (ZE) .
Therefore, the first quartile (Q1) ADC, GLCM correlation, and GLCM homogeneity remained the features of final selection .
This study is the first to demonstrate the value of ADC map TA in predicting MIBCCRT response .
TFs from ADC maps may improve prediction of CRT response and may serve as novel imaging biomarkers to optimize selection of eligible patients for induction of CRT .
