Clinically, should endovascular thrombectomy be performed in patients with acute ischemic stroke (AIS) due to large vessel occlusion (LVO) with early large infarcts (ie, low initial Alberta Stroke Program early CT score [ASPECTS ≤ 5]) (EVT) is currently the most relevant unsolved mystery in acute stroke treatmen.
Studies have shown that collateral circulation helps prolong the maintenance time of the ischemic penumbr.
Recently, published in the European Radiology journal, we evaluated the effect of collateral circulation status on patient prognosis in AIS patients with large baseline stroke lesions treated with EVT, providing an important reference for the implementation of early interventio.
This study analyzed consecutive patients with acute ischemic stroke and ASPECTS ≤5 due to large vessel occlusion in the anterior circulatio.
A total of 139 patients (age, 61±28 years; male, 58%) admitted between March 2012 and December 2017 were included in the stud.
This study demonstrates that in patients with AIS with large infarct cores (ASPECTS ≤ 5) due to LVO, good collateral circulation as determined by the FVH-ASPECTS scoring system is associated with good clinical outcomes and can identify this patient population patients most likely to benefit from revascularizatio.
