From November 17th to 20th, the 2022 Chinese Oncology Congress (CCO) was successfully held
in Hangzhou.
With the theme of "Cancer Prevention and Treatment, Winning in Integration", this conference focuses on cutting-edge views such as integrated cancer medicine, precision medicine, and translational medicine, and builds a platform, discusses scientific research, seeks innovation, and seeks common strategies
for "life and health".
During the conference, Yimaitong was honored to invite Professor Xu Quan from the Cancer Hospital of the Chinese Academy of Medical Sciences to share with us the concept of early diagnosis and treatment of gastric cancer and the importance of
tumor science popularization.
Professor Xu Quan
Ph.
D.
, Deputy Chief Physician of the Department of Pancreatic and Gastric Surgery, Cancer Hospital of Chinese Academy of Medical SciencesDeputy Secretary-General and Member of the Abdominal Cancer Professional Committee of the Chinese Medical Education Association
Deputy Secretary-General of Health Science Popularization Branch of China Association for the Promotion of International Exchanges in Healthcare
Member of Pancreatic Disease Branch of China Association for the Promotion of International Exchanges in Healthcare
Member of the Standing Committee of the Science Popularization Committee of the Chinese Anti-Cancer Association (CACA).
As the project leader or main member, he undertook 6 national, provincial and ministerial projects, and 5 institute projects
He has published 18 papers in academic journals at home and abroad, and participated in the editing and translation of 5 monographs
Gastric cancer is a serious threat to the life and health of our people, the prevention and early screening of gastric cancer is extremely important, as an expert in this field, how do you think this work should be better carried out in the future?
Professor Xu Quan
Cancer has become a major public health problem facing China, among which gastric cancer is particularly difficult
.
About 40% of the world's gastric cancer occurs in China, regardless of the incidence rate of tumors or mortality, gastric cancer ranks among the top three
in China.
In addition, China's population base is large, so the overall tumor burden of gastric cancer is extremely heavy
.
In the prevention and screening of gastric cancer, we currently emphasize the shift of the focus of tumor prevention and treatment
.
First of all, we should do a good job in primary prevention, starting from the etiology of tumors, combined with popular science methods, and do a good job in tumor prevention
.
In addition, for people at high risk of gastric cancer, secondary prevention is the key to improving prognosis, early diagnosis and early treatment, and timely control of the disease using the
current developed treatment methods.
If the disease has progressed and it is diagnosed with intermediate or even advanced gastric cancer, then the patient should receive standardized treatment in a formal medical institution and achieve maximum benefit through strategies such as multidisciplinary diagnosis and treatment, which is also the key point of tertiary prevention
.
At present, the treatment of gastric cancer has made great progress compared with the early stage, but the comprehensive prevention and treatment of cancer can not only focus on treatment, prevention and screening are also extremely important
.
To make the prevention and treatment of gastric cancer to a higher level, we should further promote the concept of primary prevention and secondary prevention, so that more people can understand the causes and symptoms of gastric cancer, which can not only enable people to change their lifestyle, avoid the pathogenic factors of gastric cancer, reduce the incidence of gastric cancer, but also help patients seek medical treatment in time, and truly realize the early diagnosis and treatment
of gastric cancer.
Yimaitong: Compared with the previous treatment methods, the combination of internal medicine, surgery and radiotherapy has significantly improved the survival of gastric cancer patients, how do you think the concept of comprehensive treatment should be better implemented in the clinic?
Professor Xu Quan
In recent years, the treatment of gastric cancer, especially in the field of drug treatment, has made great progress, and the application of new strategies such as targeted therapy and immunotherapy has greatly improved patient survival
.
This provides clinicians and patients with more treatment options, but how to make better use of these weapons and how to bring higher benefits to patients through reasonable platoon is an issue
that must be treated with caution for clinicians.
To solve this problem, clinical practice should start from three aspects
.
First of all, the doctor's treatment choice should refer to the diagnosis and treatment norms and treatment guidelines, which can largely avoid the doctor's professional tendency and treat
from the perspective of evidence-based medicine.
We have also been sparing no effort to promote the CACA gastric cancer guidelines, hoping that every clinician engaged in the treatment of gastric cancer can benefit from it and provide standard treatment recommendations
for every gastric cancer patient.
Secondly, multidisciplinary consultation should go deep into the diagnosis and treatment of
tumor patients.
Gastric cancer is a highly heterogeneous and complex disease, and a single discipline is inevitably unable to do so, so the strength of the team should be integrated, so that internal medicine, surgery, radiotherapy, imaging, pathology and other related departments can participate in the treatment discussion of patients to formulate personalized treatment plans
with the greatest benefit to patients.
Finally, the treatment of tumors has entered the era of precision treatment, and accurate drug selection is inseparable from accurate detection support, which is particularly important
for complex tumors such as gastric cancer.
Therefore, the driver gene and related target detection of gastric cancer patients should be further popularized clinically, and on this basis, targeted targeted or immunotherapy should be carried out for gastric cancer patients, which is also a strong help
to improve the overall treatment effect of gastric cancer.
Yimaitong: The "Healthy China 2030" plan outlines that China's overall cancer survival rate should be increased by 15%, what role do you think oncology science popularization can play on the road to achieving this goal?
Professor Xu Quan
Improving the overall 5-year survival rate of cancer by 15% is a daunting task, and achieving this goal requires a lot of sweat, so it is both a hope and a challenge
.
In addressing this challenge, the role of oncology popularization work cannot be underestimated
.
As we all know, the treatment effect of tumors of different stages varies greatly, and the overall treatment effect of early tumors is much better than that of advanced tumors, which is an important feature of
tumors.
Therefore, in order to improve the overall treatment effect of cancer, it is important
to eliminate the tumor at an early stage through early diagnosis and early treatment.
Taking gastric cancer as an example, Japan and China are both East Asian countries, which are similar in terms of region and culture, and Japan also has a high incidence of gastric cancer
.
However, the overall treatment effect of gastric cancer in Japan is better than that of China, and an important reason for this is that the proportion of early gastric cancer in Japan can reach about 70%, while this figure in China is only 20%.
。 Therefore, China can learn from some gastric cancer prevention and control strategies in Japan, such as publicizing gastric cancer science knowledge, so that the public can understand the high-risk pathogenic factors of gastric cancer, so as to change their lifestyle; Another example is to increase the popularization of knowledge about Helicobacter pylori, so that patients with Helicobacter pylori infection can actively receive treatment; In addition, we can also actively publicize the necessity of gastroscopy, and at the same time introduce painless gastroscopy and other means to dispel the public's fear of gastroscopy, improve the acceptance of gastroscopy, and enable more gastric cancer patients to be diagnosed at an early stage, treated in the early stage, and recovered in the early stage
